2. Complete the Application Form attached to the PDS in BLOCK LETTERS.

• Make cheques payable to "Smallco Investment Fund" and crossed "not negotiable" (if you wish to transfer funds electronically, please contact Craig Miller on (02) 8256 1000).

Applications are generally processed daily. Applications reaching the manager by 4pm on a business day, will have the calculated unit price for that day applied.

3. Instructions on completing the application form

1. Applications must be in the name(s) of natural person(s), companies or other legal entities acceptable to the manager, up to a maximum of three names per application.
2. Examples of correct names and account designations to use on your forms are:
